What triggers it
FilingIndicators.csv has the wrong header or wrong number of columns (e.g. spaces in 'templateID, reported', a trailing comma, or an extra column).
A row in FilingIndicators.csv does not have exactly two cells.
Templates: FilingIndicators.csv. Columns: templateID, reported
How to fix it
Header must be exactly 'templateID,reported'; one row per template B_01.01 ... B_99.01; no extra columns.
Two columns only: templateID,reported.
